With this upcoming Arms Plot all families are require to fill in the de register form from the PCMF so we are aware who is leaving and thus we can send-off their notes to the NHS accordingly. This is also important so we can update their records and pass the information to Health Visitor has their need to forwarded the child under age 5 years old back to UK and an update address is require to do this.
